Vaccines available:
Moderna (for individuals 18 years old and over, requires a 2nd dose 4 weeks after the first shot)*
(*Second shot will need to be scheduled at the Health Department)

Johnson & Johnson (for individuals 18 years old and over, single dose vaccine)

Pfizer (for individuals 12 years old and over, requires a 2nd dose 3 weeks after the first shot)

Please download and fill out the “COVID Vaccine Form 7-7-21” in advance.

Search for “Spencerville, OH” and follow the on screen prompts to make an appointment
